What the Program Provides
1. Annual LMAP (Life Management Assessment Plan)
A structured, interdisciplinary assessment that evaluates the beneficiary’s current medical, psychosocial, environmental, and support needs. The LMAP establishes clear goals, documents risk factors, and identifies required services. It is updated annually to reflect changes in condition or circumstance.
This program begins with a fixed number of yearly hours, determined collaboratively by the Solutions for Dignity® team and the trust, to provide routine maintenance and oversight.
2. In-Person Visits and Structured Monitoring
At least three in-person visits are conducted each year, with each visit professionally documented. Frequency may increase based on client condition or level of risk.
3. Clinical, Psychosocial, and Environmental Oversight
A dedicated Life Care Expert™ provides integrated support across care domains, ensuring that medical, behavioral, housing, and social needs are continuously evaluated and aligned with the client’s well-being.
4. Crisis Containment and Emergency Response
When a crisis arises, our team is already familiar with the client and their context. This allows for rapid response, effective intervention, and continuity of care without unnecessary delays or disjointed decisions.
